High School Registration Application

Once the online Masconomet Regional High School Registration Application is submitted, you will need email additional supplemental documents to the High School Registrar. Please take note of the list below. All documents must be emailed to Kristy Jennings at kjennings@masconomet.org

 

Required Documents for Newly Enrolled Students

Proof of Residency
Review the list of approved Residency items and submit one document from each column, A, B and C. These documents must have your name and current address on them.

Student Identification
Email a clear photo of your students Birth Certificate or Passport

Parent Identification
Email a clear photo of your Driver’s License or Passport

Current Physical and Immunization Records

If Applicable

IEP or 504 Plans
Email the most recent copy of your students plans.

Custody/Legal Guardian Official documents
Email any court documents that the school should have on record.
